There are no changes to the Primary Care Case Manager (PCCM) program. The formatting of the PCCM Medicaid State Plan changed, but the information and program remains the same. The PCCM portion was submitted to CMS on January 7, 2019, and was approved along with the Patient-Centered Medical Home (PCMH) on February 28, 2019. Although there are no programmatic changes to the PCCM program, the formatting has changed and is being promulgated. (EXHIBIT G)
